Founder, Dr. Diedra R. How do I schedule a Private Physical Therapy or Pilates appointment?

Where are you located?
We currently see clients in Santa Monica, Brentwood, West Los Angeles, and Pacific Palisades.
What are your hours of operation?
We are open by appointment Monday – Saturday. Our practitioners tailor their schedules to match yours to the best of their ability. Please contact us to schedule an appointment.
PHYSICAL THERAPY
What should I bring to my first appointment?
Please complete the new patient forms on our platform. Bring your physical therapy referrals, imaging reports, or blood testing from your referring medical professional.
Do I need a referral from my approved healthcare provider to receive physical therapy?
In the state of California, individuals seeking physical therapy are entitled to twelve (12) physical therapy treatment sessions (including the initial evaluation) without a referral from an approved healthcare provider. However, if the physical therapist determines that your condition would benefit from a referral to another health care provider prior to your twelfth sessions, your therapist may suspend your treatments until such recommended consultation occurs.
Your insurance provider may require pre-certification and a prescription in order to reimburse you for our services. Please contact your insurance provider for their requirements.
Will Monarch Wellness bill my insurance company for PT services?
In order to provide the results, and one-on-one care that our clients have come to expect, MONARCHWELLNESS is an out of network provider with all health insurance companies. Although payment for service is due at the time services are rendered, we are happy to provide you with detailed service receipts (super bills) for your submission to your insurance company. This means that claim submission and processing is the responsibility of the patient/client. We encourage you to contact your individual insurance plan to find out what your “out-of-network” benefits are, as they can vary significantly from plan to plan. Please Contact us for details.
How should I dress for my appointments?
Can I use my Flex-Spending or Health Savings Account (HSA) to pay for my physical therapy or wellness services?
Physical therapy and physical conditioning services are typically reimbursable by FlexSpend or HSA plans, when provided by a professional PT. Contact your FSA provider for details on their procedures for your successful submission.
I have a prescription for physical therapy on another facility's stationary. Do I need a specific referral to use Monarch Wellness for PT?
Do you accept Medicare?
Monarch Wellness is not enrolled as a Medicare provider and therefore can not see Medicare recipients for PT services. If you do have Medicare PT benefits and a prescription for physical therapy, we will recommend you to a different clinic so you are able to take advantage of your benefits for treatment of your specific diagnosis.
We will be happy to see Medicare eligible clients for non-covered “Wellness PT/Pilates” concurrent with your PT treatment at a different facility or after your Medicare benefits have been exhausted. Please let us know if you are a Medicare beneficiary so we can advise you appropriately.
WELLNESS PROGRAMS
Our wellness services including: personal training, restorative exercise, physical conditioning, nutritional counseling, medical guidance and acupuncture DO NOT require a prescription, are not subject to the same legal requirements and may not be reimbursable with your insurance carrier, please enquire for details. We are eager to help you return to life/sport/performance, which is why we provide one-on-one sessions focused on your individual needs. In the event you are unable to keep your scheduled appointment time, we require a 24-hr. advance notice. Cancellations with less than 24-hr. notice will incur cancellation fee equaling 100% of the scheduled treatment session.
PRIVACY POLICIES
Our studio complies with HIPAA standards, as a large part of our services depend on knowing some past or current medical history. Note that we are a teaching facility, so there periodically may be students observing your session or class. Please be assured that they are required to adhere to our published privacy policy.
SERVICES
PHYSICAL THERAPY (55 minute sessions)
Physical therapy is intended to restore and normalize physical function that has been compromised due to pain, injury, surgery, habitual behaviors or chronic conditions. The Monarch Method to rehab is a holistic approach that includes a full body initial evaluation followed by individualized care plans. We provide our clients with a wide range of one-on-one physical therapy services and offer Pilates based rehabilitation when recommended by your doctor. No referral required.
Note: Monarch Wellness Inc. is an out of network provider for all health insurances, which enables us to provide the client centered one-on-one care our patients have come to expect. We are pleased to offer a discounted cash rate for our services. We will verify your coverage and will submit your claim to your insurance company for processing on your behalf at no extra cost to you.
PRIVATE WELLNESS - PHYSICAL CONDITIONING (55 minute sessions)
Private sessions that are uniquely designed to enhance your physical fitness condition. Our whole body approach to wellness features a full physical assessment including state of the art body composition testing and analysis. Each subsequent session features hands on techniques, strength training, flexibility exercises, core conditioning, balance and agility, sport specific exercises and much more.
Workplace Wellness (customized)
Our workplace wellness programs are the solution for small busineses who understand the benefits of a healthy workplace. Studies have shown that employee wellness solutions lower absenteeism, increase job satisfaction and work productivity, improve employee retention, and lower health care costs. We offer tools such as biometric analysis, health risk assessments, prevention screenings, work-station analysis, in-house exercise programs, lunch and learns, individualized coaching, and much more.
